Getting Ready for Your Appointment

Whether you’ve set up an appointment for yourself or someone in your family, Dr. Milward and the rest of our team are excited to welcome you to our Clifton Park dental office! Our goal is to give you and your loved ones smiles dazzling smiles that last a lifetime. To achieve this, it’s important to make sure that your dental experience starts off on the right foot. On this page, we’ve listed the most important things that you should know before your first visit; call us if you’d like additional details or have a question.

Your First Visit

Our front-desk team will gladly help you check in before you make yourself comfortable in our waiting room. Once your appointment begins, our team will keep you comfortable while they check your teeth and gums for problems. They’ll ask about any current concerns or smile goals you might have, and they’ll explain their findings so that you’re fully aware of any oral health issues that have developed. We’ll work with you to plan future appointments depending on what we find during our exam, and we’ll remove any plaque and tartar we find.

Patient Forms

Before your first appointment, you’ll need to complete a series of forms so that we have your basic patient information and medical history on hand when figuring out the best treatments for your situation; you’ll also need to fill out a disclosure form for treatment, payment, and operations as well as a records release form. All of these forms can be downloaded via the links below, so you can fill them out at home instead of trying to complete them quickly at our dental office.

Dental Insurance

There’s no denying that dental insurance can be very confusing, but it doesn’t have to be at our dental office. You can call with your insurance information or bring in your insurance card to our dental office, and we’ll go over the specifics of what coverage you’re entitled to and what your co-pay/deductible will look like. We participate in a number of different plans so as to help our patients get the absolute most out of their benefits.
